Shine Summer Day Camp - Parent Contract
  • CHILD PLACEMENT CONTRACT

    This contract is required of all licensed child care centers by R400.5105b of the Michigan Administrative Code. The Michigan Department of Consumer and Industry Services is required to inspect the child care center and enforce the contract based on the terms provided in this contract.

  • ...in Shine Summer Day Camp, licensed by the State of Michigan. I certify that my child(ren) will be at least 57 months old by their first day of camp and can be enrolled in a school age program (K-7th grade).
  • Cancellations:
    I understand the one-time $40.00 registration fee and $40 per child / per camp week deposit(s) are non-refundable. I agree that there will be no refund for children who are cancelled less than 1 week prior the registered week (8:00 a.m. Monday). Cancellations must be in writing and given to the Day Camp Director, or emailed to the Day Camp Registrar at shinedaycamp@wardchurch.org.

    I agree to make full payment for the registered week(s) no later than one week prior (the Monday before) based on the published fee for that week.

  • Check-In and Release:
    I acknowledge that Shine Summer Day Camp will release my child(ren) to only those persons authorized on the Emergency Contact Card, or if a parent has sent a written, signed and dated note stating the emergency and who the child may be released to.  I understand I must check-in and check-out my child each day at the Shine Summer Day Camp Desk.


  • Termination:
    I acknowledge that Shine Summer Day Camp may terminate this agreement without notice if 1) my child’s continued participation in the program creates a direct threat to the safety of our child or other children, or Shine Summer Day Camp staff, or 2) tuition and/or other fees are not paid when due.


    This contract constitutes the entire agreement among the parties to it and supersedes any prior understandings or agreements. Each party acknowledges and states that no representation, inducement, or condition not set forth in the contract has been made or relied upon by either party.


    The laws of the State of Michigan shall govern this contract.

  • Lunch / Snacks
  • Each child will receive two snacks daily. The snack menus will be posted at the Shine Summer Day Camp desk. Please be sure to indicate any food allergies on your child’s health form.

    Parents will provide lunches for their child Monday through Friday.

  • Transportation:

    I give permission to Shine Summer Day Camp for my child to be transported to and from scheduled field trips during the week(s) my child is enrolled.



  • Emergency Permissions:

    Should a medical emergency arise, the leaders have my permission to obtain any necessary medical care for my child.



  • Photo Release:

    I give permission to Shine Summer Day Camp to use my child’s photos on social media, their website, and in promotional materials.

    I agree to defend and indemnify Ward Evangelical Presbyterian Church, its employees and volunteers against any claim or action that might arise on behalf of myself or my child, other than for the willful, wanton or reckless misconduct of Ward Evangelical Presbyterian Church, its employees or volunteers.


  • Shine Summer Day Camp as a licensed child care facility, will provide the following provisions of the Michigan Administrative Code as required by R 400.8146
  • A center’s licensing notebook will be available for parent’s review. It includes

    i) all the licensing inspection and special investigation reports and related corrective action plans since May 28, 2010

    ii) the licensing notebook is available to parents during regular business hours

    iii) licensing inspection and special investigation reports from at least the past 2 years are available on the child care licensing website at www.michigan.gov/michildcare.

    (2) A licensee shall have the following administrative responsibilities regarding staff:

    4) A criminal history check using the Michigan department of state police internet criminal history access tool (ICHAT), or equivalent, plus FBI fingerprints…shall be completed before making an offer of employment to that person. A copy of the ICHAT shall be kept on file at the center.


  • Upon signing this agreement, the parent, legal guardian or responsible adult and the child care facility agrees to abide by all of the provision contained in this contract.
